Individualized Care Plans
Our full-time social services department has the resources to make the transition as easy as possible for both resident and their families. Together we can navigate Medicare/Medicaid requirements, as well as, each individual's overall health and well being. We take into account the many different aspects of a personal care plan and assure that the resident's medical, social, and spiritual needs are met. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs.
Our unrivaled care options include an experienced therapy team that offers a wide range of short-term and long-term services personalized according to each individual's goals.
In our physical therapy programs, residents can increase mobility, muscle strength, joint motion, and balance while speech therapy reconstructs communication skills, problem-solving skills, cognitive ability, and swallowing ability. In our occupational therapy programs, residents regain daily tasks such as mobility, eating, exercising, cooking, dressing, and personal care.
